Upper Eyelid Surgery (Blepharoplasty)
As we age, the tissue beneath the eyebrow and rim of the upper eyelid loses volume, as fat disappears. This is why the excess, sagging skin you see can look a lot like a deflated balloon. An earlier technique for upper blepharoplasty emphasized aggressive removal of this excess skin and removal of fat from the upper eyelids. Unfortunately, this can leave a bony, sunken, and hollowed look to the upper eye, which is the antithesis of a youthful eyelid. Take a look at any young model in a magazine – you will see that the upper eyelids are full, and there is very little gap between the brow skin and eyelid.
To maintain youthful fullness, I either conservatively trim eyelid skin and retain the natural fat of the eyelid, or combine limited excision of upper eye skin with a fat transfer procedure in the superior orbital rim (the hollow above the eye) to create a more youthful appearance. I also may need to address sagging that is occurring on the outside or lateral edges of the brow through a brow lift. Sagging of the outer brow can greatly affect upper eye appearance, and I often find that patients who come in for eyelid surgery also need some surgical correction of the brow to achieve the appearance they want. This is different than a typical brow lift, where the entire brow is lifted. I find that most people don’t need the central portion of the eyebrow lifted, only the outer corner.
When the upper blepharoplasty is a simple trim of skin, I can often perform this in my office under local anesthesia in under an hour. When fat transfer or brow correction is needed in addition to upper blepharoplasty, I perform this procedure in an accredited outpatient surgery center. Incisions are made within the folds of the upper eyelid so that they are not visible after surgery.
Lower Eyelid Surgery (Blepharoplasty)
Again, as with upper eyelid surgery, my focus here is on restoring volume, not simply removing skin and tissue. When performing lower eyelid surgery, I want to avoid simply removing the fat below eyes and trimming excess skin. This can all too often create a “sunken in” look. Everyone has discrete “pads” of fat under their eyes, but usually they are covered by a thicker, smoother layer of fat in the midface that masks it. When this covering layer is lost, that is when the under eye fat pads become visible, and can lead to a puffy under-eye appearance.
My philosophy is to focus first on restoring the lost fat in the midface. Much of lower lid aging is caused by deflation of midface fat deposits, which then exposes bulging fat pads below the eyes. When performing lower lid blepharoplasty, I will generally remove a conservative amount of excess fat pads and create a sling to lift the sagging muscle surrounding the eye. This technique provides increased support for the lower eyelid and masks the fat pads.
Lower eyelid surgery is performed at a local surgery center. I use a lash line incision that is hidden once fully healed. I work carefully to craft a naturally rejuvenated appearance without the telltale signs of eyelid surgery such as an altered shape to the eye.
Combining the Procedures
Many people find that they have unwanted signs of aging in both their upper and lower eyelid areas. I can combine the procedures in this case for a more comprehensive rejuvenation. In some instances, eyelid surgery may be combined with non-surgical rejuvenation treatments, such as BOTOX® Cosmetic, to treat wrinkles and aged skin on the outsides of the eyes or between the brows.
